The questions asked below help to assess your health and well-being. These questions are about you, your family, friends, neighborhood, school and community. How you answer these questions will remain confidential between you and your instructor (unless an emergency arises). For each asset, there is a description and a list of statements. For each of the statements, please circle how often the statement is true for you. At the end of each section, count the number of times you chose each answer and enter the total. When you finish, enter all the scores in the summary section.
